New Medicare GLP-1 Bridge Program Now Available


New Medicare GLP-1 Weight Loss Program Now Available

Beginning July 1, 2026, Medicare introduced the Medicare GLP-1 Bridge Program, which allows eligible Medicare beneficiaries to access certain GLP-1 weight loss medications for just $50 per month.

Who May Qualify?

You may be eligible if you:

  • Have Medicare Part D prescription drug coverage.
  • Are not already eligible for GLP-1 medications through your current Medicare drug plan.
  • Do not have Type 2 diabetes, moderate-to-severe sleep apnea, or fibrosis.
  • Meet certain Body Mass Index (BMI) requirements along with qualifying health conditions, such as prediabetes, heart disease, chronic kidney disease, or difficult-to-control high blood pressure.

Covered Medications

The Medicare GLP-1 Bridge Program currently includes:

  • Foundayo®
  • Wegovy®
  • Zepbound® KwikPen® (single-dose pens and vials are not included)

What Does It Cost?

Eligible participants pay $50 per month for a one-month supply, regardless of income. This cost does not apply toward your Medicare Part D deductible or annual out-of-pocket maximum.

Medicare Part D Changes Are Helping Patients—but Creating New Challenges for Local Pharmacies

The Medicare Part D prescription drug program underwent some of the most significant reforms in its history over the past year. Much of the attention has focused on the benefits for Medicare beneficiaries, including lower out-of-pocket costs and the new annual prescription drug spending cap. While these changes are providing welcome financial relief for many seniors, they are also creating new operational and financial challenges for community pharmacies.

To better understand the real-world impact, we spoke with a local independent pharmacist who serves a large Medicare population.

Medicare Patients Are the Heart of Many Community Pharmacies

For this pharmacy, Medicare beneficiaries make up approximately 60% of its customer base and account for roughly 20% of its revenue. These patients rely on the pharmacy not only to fill prescriptions but also to answer questions about their medications, insurance coverage, and Medicare drug plans.

As Medicare policies change, pharmacies often become the first place patients turn for guidance.

Lower Drug Costs Are Making a Difference

One of the most noticeable changes has been improved affordability for many brand-name medications.

According to the pharmacist, reductions in patient costs for certain brand-name drugs have been a positive development, helping both patients and improving prescription affordability. Patients have generally experienced lower prices compared to previous years, making it easier for many to stay on their prescribed therapies.

These improvements reflect Medicare's recent efforts to reduce prescription drug expenses and place a cap on annual out-of-pocket spending.

Patients Still Need Help Understanding Their Coverage

Although costs have decreased for many medications, Medicare Part D has not necessarily become easier to understand.

The pharmacist noted that many patients continue to struggle with deductibles and understanding how their drug coverage works throughout the year. When questions arise, patients are often directed to Medicare.gov or encouraged to seek additional guidance regarding their specific plan options.

This highlights an ongoing need for education during Medicare's Annual Enrollment Period, when beneficiaries have the opportunity to review and compare their prescription drug coverage.

Behind the Counter, Financial Pressures Continue

While patients are benefiting from lower prescription costs, independent pharmacies are facing increased financial pressure.

The pharmacist reported that reimbursement delays can extend from 30 to 45 days, creating significant cash flow challenges for small businesses. In addition, reimbursement rates on many prescriptions continue to decline, making it more difficult for independent pharmacies to remain financially sustainable.

Unlike large national chains, independent pharmacies often have fewer financial resources to absorb delayed payments or below-cost reimbursements.

Access to Care Depends on Healthy Pharmacies

Perhaps the greatest concern expressed during the interview was the long-term effect declining reimbursements could have on patient access to care.

The pharmacist believes current reimbursement rates are negatively affecting community pharmacies and, ultimately, limiting patient access to medications and personalized pharmacy services. As reimbursement pressures increase, the ability of independent pharmacies to continue serving rural and underserved communities may become increasingly difficult.

The Bottom Line

Recent Medicare Part D reforms are accomplishing one of their primary goals: helping many beneficiaries afford their medications. Lower drug costs and capped out-of-pocket expenses are meaningful improvements for seniors living on fixed incomes.

However, the interviews also reveal another side of the story. Community pharmacies are working harder than ever to help patients navigate these changes while simultaneously managing reimbursement delays and shrinking margins.

As Medicare continues to evolve, maintaining a balance between affordable medications for patients and sustainable reimbursement for the pharmacies that serve them will be essential. After all, lower prescription costs only matter if patients continue to have convenient access to trusted local pharmacies.
